After surviving Blue Lock's ruthless selection process, Yoichi Isagi and the other remaining players must fight for the project's existence against Japan's Under-20 national team. Standing in their way are the so-called diamond generation and the internationally recognised genius Sae Itoshi. Victory would allow Blue Lock to take over the U-20 side; defeat would bring the entire project to an end. The fourth stage adaptation turns that decisive confrontation into a live theatrical contest of ambition, teamwork and individual brilliance.

Ryohei Takenaka plays Yoichi Isagi, Ryono Kusachi plays Rin Itoshi, Nobunaga Sato plays Meguru Bachira and Takamichi Sato plays Seishiro Nagi. They are joined by Gaku Matsuda, Yuya Uno, Kosuke Asuma, Ryo Saeki, Yo Maejima, Shuji Kikuchi, Kairi Miura, Yu Imari, Kento, Dai Isono, Koki Muramatsu and Shojiro Yokoi. The opposing side and supporting cast feature Haruto Sakuraba, Gaku Matsumoto, Joel Shohei, Tatsuya Kageyama, Toshimasa Nakadoi, Ren Furuta, Daiya Aramaki, Kenzo Doi and Hiroki Ino as Sae Itoshi.

Written and directed by Naohiro Ise, with a theme song by Kradness, the production ran at THEATER MILANO-Za in Tokyo from May 15-25, 2025 and at Higashiosaka Cultural Creation Hall's Dream House Main Hall from May 30-June 1. The adaptation is based on Muneyuki Kaneshiro and Yusuke Nomura's manga.

The set contains the approximately 120-minute main performance and a separate approximately 130-minute bonus Blu-ray. Extras include making-of footage, a collection of scenes that changed between performances, after-talk highlights and curtain calls. Both discs use Linear PCM stereo, with Japanese audio for the main performance and no subtitles listed for the edition. A 16-page booklet is included.
