TsumaSho: If My Wife Becomes an Elementary School Student explores how a grieving family learns to move forward when an impossible reunion changes their lives. Ten years after losing his wife Takae, Keisuke Niijima remains deeply affected by her death. One day, a schoolgirl named Marika Shiraishi appears and says she is Takae. Yayu Murata's story of family renewal becomes an animated drama, with Daisuke Hirakawa as Keisuke and Aoi Yuki voicing both Takae and Marika.

Volume 5 brings together episodes 9 and 10. After recovering her memories, Takae discovers that what has happened is not reincarnation. She asks Chika to call her Marika so the child will not be left in a difficult situation when she returns. Keisuke also begins to understand the truth, although accepting it is painful. After Moriya confesses her feelings, he tells her about Takae and explains why he feels unworthy of her affection. While he struggles with the idea of parting for Takae's sake, Takae visits a wedding venue with Mai and Renji and gathers the courage to make a request.

The disc contains approximately 46 minutes of episodes, commentary on episode 10 by Daisuke Hirakawa and Kana Okazaki, and the creditless ending sequence. A special booklet accompanies Narihito Sekikawa's newly illustrated jacket. This region-free Blu-ray features Japanese Linear PCM audio and no subtitles.
