After receiving an Eyecon sent by his late father, Takeru is defeated in battle against the Gamma. In the space between life and death, he encounters a mysterious hermit who gives him the power to return to the living world. With the Eyecon and a transformation Driver, he becomes Kamen Rider Ghost. He has only 99 days to gather the Eyecons of 15 heroes and regain his life, fighting for the world and its people as well as his own future.

The seventeenth Heisei Kamen Rider series aired on TV Asahi from October 2015 to September 2016. This final collection contains all 13 concluding episodes: episodes 38-44 on the first Blu-ray and episodes 45-50 on the second.

The cast features Shun Nishime, Hikaru Osawa, Ryosuke Yamamoto, Takayuki Yanagi, Yoshiyuki Morishita, Hayato Isomura and Naoto Takenaka. Based on the original work of Shotaro Ishinomori, the series credits producers Motoi Sasaki of TV Asahi and Kazuhiro Takahashi of Toei, screenwriter Takuro Fukuda, and directors Satoshi Morota and Kyohei Yamaguchi.

A third Blu-ray contains the final chapter of the spin-off drama Alan Hero Legend, the Eyecon Talk round-table discussion, cast comments, trailers and promotional clips, broadcast junctions and an original design gallery.

The three-disc collection presents 293 minutes of television episodes in 16:9 1080p with Japanese Linear PCM stereo audio. The supplementary disc is also in Japanese. This edition has no subtitles.
